Position Overview

We are seeking an experienced **Medical Director I** to provide comprehensive primary care and clinical leadership for a senior-focused, value-based care program.

The Medical Director I will conduct comprehensive patient assessments, develop individualized care plans, coordinate care with an interdisciplinary team (IDT), and provide clinical direction and support to Nurse Practitioners and other members of the care team.

This is an excellent opportunity for a physician who enjoys building strong patient relationships while playing a leadership role within a collaborative clinical environment.

Key Responsibilities

* Conduct comprehensive in-person assessments of participants. * Provide primary care, medical assessments, treatment planning, and ongoing medical coverage. * Collaborate with the interdisciplinary team (IDT) to develop individualized care plans. * Coordinate patient care and discharge planning with hospitals, nursing homes, specialists, and community-based physicians. * Oversee participant care in nursing homes and hospital settings. * Coordinate acute, short-term, and long-term placements. * Participate actively in IDT meetings and communicate changes in participant conditions. * Help coordinate 24-hour care delivery. * Make regular nursing home visits based on participant needs. * Provide clinical direction and support to Nurse Practitioners and other IDT members. * Help develop and maintain a strong clinical team and positive care culture. * Promote high-quality, patient-centered, value-based care.

Required Qualifications

* Successful completion of an **ACGME-accredited residency program**. * Master's or Doctorate degree. * Board certification in: * Internal Medicine * Family Medicine / Family Practice * Emergency Medicine * Geriatric medicine experience or certification preferred. * Active, unrestricted physician license in the applicable state. * Active state CDS certification. * Active DEA registration. * Current knowledge of chronic care and geriatric medicine preferred. * Currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants. * Must be willing and able to participate in rotating night and weekend on-call coverage.

Ideal Candidate

The ideal candidate is a physician with **2–5 years of clinical experience** who has strong knowledge of primary care, chronic disease management, and geriatric medicine.

Candidates with experience working in an **Interdisciplinary Team (IDT), PACE, value-based care, nursing homes, skilled nursing facilities, hospitals, or senior-focused healthcare programs** are particularly relevant.

Leadership or supervisory experience with **Nurse Practitioners, Physician Assistants, or other clinical professionals** is highly desirable.
